Complete the client Assessment Pack ! To determine the type of service a client may need, the client assessment packet will need to be completed by the client or caregiver on the behalf of the client.
We help you to find the right care for you. A list of the services are on the Service tab on the website to be reviewed.
Perform Home Safety Evaluation - After the client assessment packet is completed, Miracles of Hope Care Administrator will arrange a client home visit to perform a free home safety evaluation. There we will provide recommendations regarding safety items and ensure the home has the necessities needed to provide our services efficiently.
Complete paperwork and payment agreements - All necessary forms/ documents will be required to be completed accurately. Once completed Care will begin.
